Guilford Community Care Network

Guilford Adult Health, Inc., operating as Guilford Community Care Network (GCCN), is an essential safety net organization dedicated to ensuring equitable access to comprehensive healthcare services for uninsured adults living in Guilford County, North Carolina, with incomes between 0-200% of the Federal Poverty Line. Our mission is to ensure equitable access to quality care while our vision is to create a healthcare system where everyone can achieve optimal health regardless of socioeconomic barriers.

Our programs are designed to address the diverse needs of our community through targeted interventions, community outreach, and strategic partnerships. We focus on reducing healthcare disparities, increasing awareness and education around health issues, and addressing social determinants of health that influence overall well-being. By fostering collaborations with local healthcare providers and community organizations, GCCN strives to build an inclusive and responsive healthcare network that improves health outcomes and enhances quality of life for all Guilford County residents.

Our core programs include:

• Orange Card Program – This initiative provides eligible individuals with an “orange card” (i.e. a healthcare access card) that facilitates entry into the GCCN network. It assigns patients to a medical home for ongoing primary care, including referrals to specialty and dental services, tiered care management, and comprehensive resources.
• Provider Outreach Program – This program extends specialty care through volunteer clinicians who provide services at GCCN partner sites or within their own practices. Patients pay on a sliding fee scale ($0-$60), making care affordable and accessible.
• Guilford Dental Access Program (GDAP) – GDAP offers essential dental services such as examinations, extractions, fillings, pain management, and minor restorative procedures at a flat rate of $40, with scholarships available to those in need.
